AFB Application

To join our team of qualified caregivers here at ABF, Inc. you must bring in the following items to process your application:

  • Proof of Identification: Drivers License, State or Military ID
  • Resume if available
  • Social Security Card
  • Current Auto Insurance Card/Form with Expiration Date
  • **CPR Certification---If have not taken CPR, contact your local office to setup a time to register to take the class.
  • **Yearly Physical examination
  • **Yearly TB, Mantoux, or Chest X-Ray
  • Professional Licenses, Certificates of Training if available
  • Copy of degree, diploma, GED, or transcripts
  • **Criminal Background record checks

All information must be in prior to providing care to a client.
