    Mental and physical health are closely connected. At A Brighter Future Healthcare Services, Inc., mental-health counseling is coordinated through the patient’s established primary-care relationship.

    This integrated model allows the clinical team to consider medical history, medications, physical symptoms, behavioral-health concerns, substance use, stressors, and other factors that may affect the patient’s overall well-being.

    To receive counseling services from Dr. Tim Gant, you must first become an established ABF primary-care patient and receive an internal clinical referral.

    Your Mental Health Deserves Professional Attention

    Mental-health concerns do not always appear as a visible crisis. A person may continue working, caring for family, attending school, and meeting daily responsibilities while privately experiencing anxiety, depression, trauma-related symptoms, emotional exhaustion, grief, substance-use concerns, relationship difficulties, or a growing sense that life has become difficult to manage.

    Seeking support does not mean that you are weak or incapable. It means you recognize that your mental and emotional well-being deserves the same thoughtful attention as your physical health.

    ABF’s coordinated care model begins with a primary-care evaluation. Your medical provider will assess your concerns, consider possible contributing factors, and determine whether a referral to Dr. Tim Gant or another appropriate mental-health resource is clinically recommended.

    How to Qualify for Counseling With Dr. Tim Gant

    Counseling services with Dr. Tim Gant are not available through direct self-scheduling. Patients must complete the following process:

    Step 1

    Establish Primary Care With ABF

    You must first be accepted and established as an ABF primary-care patient. New patients begin by completing ABF’s new-patient and insurance-verification process. Acceptance may depend on provider capacity, insurance participation, clinical needs, required documentation, and other eligibility considerations.

    Complete a Primary Care Evaluation

    After becoming an established patient, schedule an appointment with an ABF primary-care provider to discuss your mental-health, behavioral-health, emotional, or substance-use concerns. Your provider may review current symptoms, medical history, medications, substance use, sleep, stressors, and safety concerns.

    Step 3

    Receive a Clinical Recommendation

    Following the evaluation, the ABF primary-care provider will determine whether counseling with Dr. Tim Gant is clinically appropriate. A recommendation may include a referral to Dr. Tim Gant, continued monitoring through primary care, additional medical evaluation, or referral to an outside specialist or higher level of care.

    Internal Referral Review

    When counseling with Dr. Tim Gant is recommended, the ABF primary-care team will initiate an internal referral. The referral may be reviewed for clinical appropriateness, provider availability, patient eligibility, insurance requirements, and scheduling capacity.

    Counseling Intake and Scheduling

    Once the referral is accepted, an ABF team member will provide instructions for completing any required behavioral-health forms and scheduling an intake appointment. Submitting forms or receiving a primary-care recommendation does not guarantee immediate placement.

    Mental-health symptoms may be influenced by medical conditions, medications, hormonal changes, nutritional deficiencies, sleep disorders, chronic pain, substance use, and other physical-health factors. Beginning with a primary-care evaluation allows ABF to consider the whole person before determining the next step.

    Identify medical concerns that may contribute to emotional symptoms
    Review medications and possible side effects
    Evaluate physical and behavioral-health needs together
    Determine whether counseling is the appropriate level of care
    Coordinate treatment between providers
    Reduce fragmented or duplicated care
    Refer patients to outside specialists when necessary

    ABF’s goal is not simply to place patients into counseling. It is to connect each patient with the most appropriate form of care based on a clinical assessment.

    Conditions and Concerns We Evaluate

    ABF primary-care providers can evaluate patients who are experiencing a range of emotional, behavioral, or substance-related concerns. When clinically appropriate, qualifying patients may be referred to Dr. Tim Gant for counseling support.

    Trauma and PTSD-Related Concerns

    When appropriate, an established ABF primary-care patient may be referred to Dr. Tim Gant for trauma-informed counseling at a safe, respectful pace.

    Substance Use and Recovery

    An evaluation may include a review of substance-use patterns, withdrawal risks, and medical complications to determine the appropriate level of care.

    Grief and Loss

    When appropriate, counseling may provide a private setting to process loss without judgment or pressure to recover on a set timeline.

    Individual Counseling

    Sessions may address anxiety, depression, trauma-related concerns, substance-use recovery, stress and burnout, grief, emotional regulation, relationship difficulties, and personal development.

    Treatment is based on the patient’s clinical needs, history, strengths, goals, referral information, and progress.

    Mental Health Assessment

    After an internal referral is accepted, the behavioral-health intake may explore current symptoms, mental-health history, relevant medical history, trauma exposure, relationships, and current stressors.

    The intake helps determine whether counseling with Dr. Tim Gant remains clinically appropriate and informs the treatment plan.

    Addiction Assessment & Counseling

    For referred patients experiencing substance-use concerns, the assessment may review substances used, triggers, consequences, recovery history, and co-occurring symptoms.

    Outpatient counseling is not a substitute for emergency care, medical detoxification, or a higher level of addiction treatment.

    Telehealth Counseling

    Telehealth counseling may be available to established and referred patients when clinically appropriate and legally permitted.

    Telehealth may not be appropriate when an in-person assessment is needed, there is an immediate safety concern, or the patient requires a higher level of care.
