Prescreening & Health Insurance Verification
Before becoming a new ABF patient, please complete this brief prescreening and insurance verification. The information you provide helps A Brighter Future Healthcare Services understand the care you are seeking, verify your insurance, identify any records or clinical review that may be needed, and determine the appropriate next step in your enrollment.
After ABF reviews your prescreening, eligible applicants will be directed to Step 2: the secure New Patient Clinical Application.
Why ABF Uses Prescreening
ABF wants to make sure our outpatient services, clinical resources, scheduling model, and care-coordination capabilities are appropriate for each prospective patient's needs. Prescreening also helps us prepare for your care before your first appointment and identify anything that may need attention beforehand.
Prescreening is reviewed individually and completion does not guarantee acceptance or scheduling.
Before You Begin
- Have your current insurance card available. You will be asked to provide your insurance information and clear images of the front and back of the card.
- Be prepared to briefly describe the care you are seeking from ABF.
- Know the names or specialties of clinicians involved in your current care, particularly if you see specialists regularly.
- Have basic medication information available, especially if another clinician currently manages prescription or controlled medications.
- Be prepared to identify recent hospital, emergency, urgent-care, or major treatment activity when applicable.
- Consider whether ABF may need records from another healthcare provider before your first visit.
- Answer each question completely and accurately.
- Allow approximately 7–10 minutes to complete the prescreening.

What Happens Next?

ABF receives your insurance and preliminary care information.

ABF reviews your information.
Our team may verify coverage, review your care needs, or contact you if additional information or records are required.

Continue to the New Patient Application.
Once cleared to proceed, you will complete Step 2, the comprehensive clinical New Patient Application.

ABF completes enrollment review.
After the necessary information has been reviewed, ABF will contact you regarding scheduling and next steps.
